Principles Of Excellent Autism Care And Treatment

As an autism consultant I've observed families struggle with the overwhelming amount of information and treatment options recommended for their child. While I don't believe that any approach addresses all of a child or family's needs, I have seen the effects (both positive and negative) of enough approaches to appreciate the principles that seem to lead to the best results. In my quest to get to the 'root' of my client's obstacles to progress, I've trained and achieved certification in several complementary modalities over the past several years. This list is a summary of some of my observations of what is most helpful working with families through a variety of developmental approaches:
1. Treatment is Heart-Centered - all caregivers (parents, babysitters, therapists) and service providers should ideally be coming from a place of supportive, loving energy. (Not to be confused with celebrating for no reason).
2. Family Priorities are established in the intake process to guide treatment objectives.
3. Treatment goals are directly related to functional improvements in Quality of Life for the child and the family. (A child in Florida does not need to know what a sheep is when he doesn't know another person is in the same room with him.)
4. Spontaneous independent demonstration of skills and behavior is how progress should be measured - it does not matter what a child is doing when prompted if we don't see spontaneous generalization and use of skills.
5. Instill in parents that they don't have to do everything at once, nor do they have to do everything before the child turns 5 years old. The medical establishment unnecessarily stresses parents by drilling this false notion into their psyche. People grow and develop throughout their whole lives. Otherwise none of us would have learned to drive a car.
6. Don't believe "conventional wisdom". Science is important and critical thinking is imperative. However, be aware that it takes decades before the cutting edge research is distilled into textbooks which are used to train the next generation of therapists. The majority of professionals diagnosing autism and making treatment recommendations have little real-world experience in the homes of families who live with an autistic child. In addition, it is rare to find a diagnosing professional who has a good idea of the pros and cons of various treatment modalities. Just because a PhD or MD says ABA is the 'only' effective autism therapy does not mean it is true (it is not). It just means it is the only one they have heard much about and they probably learned about it in their doctoral or medical school program at a continuing education seminar.
7. Autism can be thought of as a disorder of Regulation - therefore, treatment priorities should emphasize 'bottom-up' as well as 'top-down' processing modalities that support regulation of the brain and nervous system. Self-regulation and Co-regulation can be addressed through a combination of various body-mind (i.e. HANDLE® and MNRI®) and cognitive-developmental approaches (i.e. RDI® and Miller Method®) that are very successful at getting to the root of many of the processing challenges children with autism tend to have.
8. Teaching skills (i.e. academics, ABA, etc.) should be addressed after Self-Regulation and Co-regulation are in progress or developing. (Self-regulation is not to be confused with "Compliance", which often happens in place of mindfulness and self-regulation.)
9. Parents should be taught as soon as possible about the concepts of regulation, co-regulation, and experience-sharing communication. Co-regulation is rarely talked about in most autism therapies, aside from RDI®. It is critical that families understand this concept, because it is a foundation to communication and independent functioning, and is often non-existent with individuals with ASD. Instead a pattern of compensation develops and 'pseudo-coordination' or 'pseudo-conversation' ensue.
10. Watch out for the tendency to 'over-therapize' the child with autism. Remember that children with autism are children first.. It is not 'normal' for a child to spend dozens of hours each week in one-sided therapeutic interactions (which much of autism treatment can be without understanding co-regulation, which is mentioned in #9). It is important that children with ASD are provided with competent roles and participate in family life like their siblings. We hope for children with autism to turn out 'normal' as adults when much of their childhood does not provide 'normal' experiences if they are over-therapized.
11. Pay attention to how the child spends his time. The brain changes itself and organizes itself through experience. This means every waking hour the child with autism is practicing and rehearsing building neural pathways, digging 'grooves' deeper and deeper. Think about the child's interests and how much time he spends engaging in repetitive, "mindless" activities (to reference Dr. Steve Gutstein), how much time he spends watching television or playing a video game. Parents can significantly alter the course of the child's brain development by becoming educated on autism deficits, and making simple adjustments to daily interactions to build opportunities for mindfulness, social engagement and creative thinking. The brain does not stop self-organizing after age 5, and parents can make a significant difference in brain development if they are aware of a few key strategies.

Autism Treatment - What Medication Should My Autistic Child Be On?

I thought I would write today about the antipsychotic medication Risperdal. Both my boys have been on Risperdal and my oldest boy is still on Risperdal.

Risperdal has just recently in October 2006 been FDA approved for the treatment of irritability in Autism. I realize that Risperdal has only been approved for children and adolescents (ages 5-16 years) and my son Jordan is 19 years old but he has been using Risperdal for years, even before it was FDA approved. It is still addressing his aggressive behavior.

I would not say that either of my boys have real behavior problems but Jordan since the age of about 10 has had problems with irritability and temper tantrums. Taking Risperdal has decreased those episodes.

This medications has been shown to alleviate behavior problems such as:

- aggression

- deliberate self-injury

- temper tantrums

- quickly changing moods

I did not realize this but on the official Risperdal website they state that Risperdal is the first medication that the FDA has approved for use in children and adolescents with autism.
It should be mentioned that there are several side effects with RISPERDAL that can possilby occur while taking this medication. They include increase in appetite, fatigue, upper respiratory tract infection, increase in saliva, constipation, dry mouth, tremor, muscle stiffness, dizziness, involuntary movements, repetitive behavior, rapid heart beat, confusion, and increase in weight.

My son Jordan takes a very low dosage of Risperdal so he does not suffer from any of these side effects except for weight gain. I think that would be my only complaint I would have about using Risperdal. He has a tendency for weight gain already and I don't think that the Risperdal helps. Every now and then I sometimes get tempted to take him off it but I know that he is much calmer and happier being on it. One thing that has been beneficial is that it helps him have a good night sleep. Jordan does not have a sleep problem like his brother Taylor has and many other individuals that have autism. That is certainly a plus!

There are other less common, though more severe side effects, these include neuroleptic malignant syndrome, tardive dyskinesia, and hyperglycemia and diabetes. If you are concerned about any of these side effects it would be best to make an appointment with your child's pediatrician and talk to them about your concerns.

Studies have shown that a significant number of very young children with ASD respond differently to images of their parents' faces. In other words, if there is an early challenge associated with recognizing the significance of parents' faces, (and the reinforcement generated from this is therefore absent) the skill of emotional reading may not develop later in life.

Autism Treatment - How to Successfully Work With a Biomedical Autism Doctor

These suggestions have been acquired over the years in my practice and have helped me assist my patients greatly. Also, in talking with many other doctors working with families of a loved one with autism these recommendations often hold true as well. Working with a doctor who specializes in biomedical intervention for autism can be a challenging process for some parents.

Understanding that a particular doctor may be extremely busy and needs your help in assisting them help your child will go a long way in the treatment of your child. Here are some recommendations.

Journal - keep a running journal of your observations and timeline of therapies you are implementing.

Keep a spreadsheet of therapies.

Keep dates of when new therapies such as when supplements were started, stopped, and what reactions were seen (good or bad).

Recognize your child's patterns - situational, seasonal, time of day.

If added new therapies and problems are seen - then cut out some or all new therapies giving before reactions occurred, then reintroduce slowly to isolate which one was the potential culprit. Notify your doctor of these changes.

Autism Treatment - Clostridia & Regression

There is an issue with regression that comes up frequently in my practice. Oftentimes, this regression will happen following a course of antibiotic treatment or following antifungal treatments, and rather than improvement, you see regression. Antifungal treatments can include prescriptions such as Nystatin, Diflucan or Nizoral or herbal remedies like oregano oil, grapefruit seed extract, etc. If you see a worsening in behavior along the lines of irritability, aggression, self injury or withdrawal, you could be seeing signs of an underlying bacterial problem.

A common bacteria that many children on the Autism spectrum deal with is called clostridia. There are many types of clostridia bacteria, one type is called clostridia dificil and can actually cause one form of inflammatory bowel disease. When we talk about these negative behavioral changes and regression, we are not talking about this type. Although that form can produce certain toxins that could contribute to these negative behaviors but we are really talking about the broad family of clostridia rather that a specific form.

Great Plains Labs has two tests called the Organic Acid Test and the Microbial Organic Acid Test which look at the specific yeast and clostridia bacteria markers. HPHPA is the clostridia marker and a metabolic toxin that is picked up on these tests. That metabolic toxin appears to interfere with brain chemicals, which then can manifest in some of those behavioral issues we see with kids on the spectrum.

Yeast and clostridia live in competition with one another in the digestive tract. So that is why we often see regression with the use of antifungals. If you are not treating them both at the same time, when one is lowered then the other one can tend to flourish. So if you are treating a yeast overgrowth without also treating a clostridia overgrowth, clostridia can get worse because it has less competition from the yeast that is dying. You can also have an increase in clostridia following antibiotic use as that can kill off the beneficial bacteria in the gut which allows more growth for the opportunistic clostridia. If you see regression in your child following a course of antibiotics or antifungals, it may be wise to look into whether a clostridia overgrowth is the cause.

Using Methylation Chemistry for Autism Treatment

Methylation is a vital biochemical reaction in the body that supports the cardiovascular, hormone, immune, and detoxification systems, DNA/RNA structure and function, and other key metabolic system controlling energy production. It is commonly a problem in many autistic-spectrum individuals. There are some effective therapies, namely methylcobolamin therapy (injection, oral, sublingual), as well as other methylation supplements such as DMG and TMG.

The two most referenced chemical reactions in this methylation system are Re-methylation and Trans-Sulfuration.

Re-Methylation (or methylation) - this pathway involves the conversion of Homocysteine to Methionine. Methionine is a rate-limiting step for the production of other necessary chemicals which affect the heart and blood vessels, muscle tissue, immune and nervous system. The conversion of homocysteine to methionine can occur by direct transference of a methyl (CH3) groups from Methylcobalamin (B12) or betaine (trimethylglycine or TMG).

Homocysteine sits at a junction of two different biochemical reactions. Because of its position in this biochemical matrix it has the capacity to impact all methylation and sulfur group transference metabolic processes in the body. The most recognized impact of homocysteine is associated with increased risk for cardiovascular disease. However, in individuals with autism its impact affects other functions as well particularly cognition including concentration, attention, and language.

Trans-Sulfuration - this pathway involves degrading homocysteine to two different amino acids - taurine and cysteine. Taurine is most commonly known for cardiac support and liver support, detoxification, bile acid formation and cholesterol excretion. Cysteine has direct influence on glutathione production. Glutathione is a potent anti-oxidant and has broad reaching effects on the DNA/RNA protection, heavy metal detoxification, and immune function. Many children on the autistic-spectrum have deficiencies of both taurine and cysteine.

There are many intermediary steps involved in these two important biochemical reactions. Envision a wheel that is constantly spinning in a clockwise fashion. Homocysteine is at 6 o'clock and Methionine is at 12 o'clock. The goal is to get from 6 o'clock to 12 o'clock, and then from 12 o'clock to 6 o'clock. This keeps the wheel spinning in the right direction. Certain other chemicals will impact upon this wheel at specific points. If any one of these intermediary steps is blocked than the wheel slows down or stops. This causes a backlog of chemical information that has deleterious effects on our overall health.

Methylcobolamin, activated Folic Acid (called methyl-folate), and Betaine (TMG) are responsible for taking homocysteine from 6 o'clock to methionine at 12 o'clock. If one system is faulty (methylcobolamin/Folic acid) than betaine (TMG) can help out. SAMe (s-adenosylmethionine) the body's "universal methyl donor" helps take methionine from 12 o'clock to homocysteine at 6 o'clock. Along the way other important chemicals are being spun off in different directions to support the many dependent biochemical reactions in the body.

The problem with many autistic individuals is that this system does not operate properly severely compromising their immune, nervous, and detoxification systems. The impact can be enormous including increased chronic infections, inability to detoxify their body of chemicals and heavy metal toxins, and neuro-cognitive problems such as language processing, attention, focusing, and concentration. There appears to be a genetic component with many autistic individuals for having a problem with this methylation and trans-sulfuration chemisty. However, for many the problem does not become manifest until their system is negatively impacted upon from nutritional deficiencies, digestive problems from yeast, bacteria, parasites, malabsorption from digestive imbalances, heavy metal toxins from vaccines or environmental exposures.

The most important way that music therapy helps autistic patients is by helping to develop their use of speech. Music helps us connect to the verbal and the non-verbal functions of our minds. Many people that suffer from autism have varying degrees of difficulty with speech. Some patients can only make humming or grunting sounds, while others have developed the ability to put together words and sentences that will at least allow them to communicate. Regardless of how much skill the patient has with speech, there is something to be gained by participating in musical therapy.

It is not unusual for autistic patients to be particularly good with music. Some have excellent pitch; others can learn to play instruments with a minimum of instruction. Even if the person doesn't show amazing musical abilities, some difficult to handle autistic patients have musical abilities that greatly exceed their other abilities. Musical therapists will use music to link this type of learning to other types of learning, including speech, social and behavioral development. This can also be used as a means of communicating emotions and to develop the memory.

Music therapy can be very beneficial to autistic patients on its own and when combined with other treatment techniques. A trained music therapist can teach a patient how to communicate using nonverbal methods, which makes it easier for autistic individuals to learn. Autism Treatment - Using Probiotics in Autism Treatment

I want to discuss the different types of probiotics and what are considered general probiotics and those that are considered treatment specific probiotics. The options on the market for probiotics are many and varied. Three major examples of all around, multi-floral probiotic products are Probiotic Support Formula from New Beginnings, Therbiotic Complete from Klaire Labs and Multi-Flora Spectrum from Kirkman labs. There are certainly other forms but these are three very good, multi-floral probiotics that have a number of good bacteria in them. And a good way to understand the benefits of these probiotics is to view them as a multi-vitamin for the overall health of your digestive tract. Probiotic Support Formula, Therbiotic Complete and Multi-Flora Spectrum are all very helpful for the overall health of the digestive system.

Next we can discuss what I call the treatment specific probiotic options. Treatment specific probiotics are used to treat a specific disorder or issue within the body. Culturelle is one example of this. What Culturelle is, is a specific type of probiotic called acidophilus GG and it can be very effective at targeting and going after clostridia bacteria. Quite a few people use Three Lac as a treatment for yeast since it is particularly helpful for that. So of course, you wouldn't use Three Lac long term as an all around, immune and digestive support aid. You would use Three Lac with the intent to help control yeast overgrowth. VSL #3 is another example of a treatment specific probiotic. It is a very potent probiotic that has been used successfully to break down oxalates. And while it does have some effectiveness against clostridia as well, for the most part it is used to help combat high oxalate levels. A fourth treatment specific probiotic is called Saccharomyces Boulardii which is a type of yeast that will attack another form of yeast called candida. So Saccharomyces Boulardii is used specifically as part of an anti-fungal approach because of this tendency to help control candida. It can also be very effective when used while you are taking antibiotics, it can prevent a yeast overgrowth from starting. Saccharomyces Boulardii also does have some effectiveness at fighting clostridia as well.

So as you can see, there are many probiotics out there and this was to serve as just a highlight. As a recap there are multi-floral probiotics which serve to enhance the immune system and to enhance the digestive system and then there are probiotics that are treatment specific that I outlined above.
